On August 24, 2018 Advantage Care Health Centers held its first Patient Focus Group. We worked collaboratively with the Freeport day program leadership and staff who support this initiative.

The goal of this focus group was to meet with our patients with developmental and intellectual disabilities as they make up the majority of our practice and are a vulnerable population. The patients represent Brookville and Freeport.

As a Federally Qualified Patient Centered Medical Home it is our goal to ensure all patients have an opportunity to provide feedback to our Services and identify improvements.  As we embark on our recertification for PCMH (Patient Centered Medical Home) with NYS DOH we value continued quality improvement and providing “patient-centered” care.  We plan on meeting every two months and are considering adding educational programs to our agenda going forward.  Our next meeting is scheduled for October 26, 2018.
