Patient Focus Group in Freeport AHRC Workforce Building!
On October 26th, 2018, Advantage Care Health Centers held its second Patient Focus Group.
The Goal of this focus group was to meet with our patients with developmental and intellectual disabilities as they make up the majority of our practice and are a vulnerable population.
As a Federally Qualified Patient Centered Medical Home, it is our goal to ensure all patients have an opportunity to provide feedback to our Services and identify improvements.
As we embark on our re-certification for “PCMH” with NYSDOH we value continued quality improvement and proving “patient -Centered” care.
We plan on meeting every two months an are considering adding educational programs to our agenda going forward.
