Our History
Advantage Care Health Centers has been committed to offering quality health care services since 2004, providing patients with access to medical, dental and mental/behavioral health services.
In 2009, Advantage Care expanded to include The Fay J. Lindner Center for Autism and Developmental Disabilities. It continues to provide comprehensive outpatient clinical services and evaluations to people with Autism Spectrum Disorder (ASD) and their families across their lifespan.
In 2011, Advantage Care became a recognized Patient Centered Medical Home (PCMH). It strengthens the physician-patient relationship by promoting improved access, coordinated care and enhanced patient/family involvement.
In 2014, our Freeport location opened, which has available hours from Monday – Saturday. Click here for more information.
In 2015, Advantage Care Health Center became a Federally Qualified Health Center (FQHC), expanding its mission to provide outpatient clinical services to all members of the community.
Advantage Care works with various community partners to enhance services by offering residency rotations to medical, psychiatric, nursing and other health-related students from the Northwell Health System.
